If a new user is going to use a phone, please submit the attached form. Keep in mind that we also need to know where the old phone user went so we can change their phone as well.
If users are simply changing desks in the same room or floor even, the user should generally take their phone with them. There are exceptions of course. For instance, a special phone is needed where the user was, etc.
Also, for each user that moves, another form should be filled out.
